HS 2021 Jr/Sr Prom

All students attending the 2021 CAK Prom must complete the Consent, Release, Indemnification and Hold Harmless Agreement which must be notarized by both parents. There are two notaries available in the high school. You can bring your form to the HS front desk and either Mrs. Key or Mrs. Harpending can notarize the form after you sign it.  This form was sent to all Jr and Sr parents and students in the March 12th email.  There will also be forms available at the HS front desk.

 

If you are bringing a guest outside of CAK, you will also need to complete the Guest/Date Agreement.  That form does not need to be notarized and will also be available at the front desk.

Prom was previously scheduled for April 24th.
